We make the referral process as straightforward as possible. From your first call to a confirmed placement, our team guides you every step of the way.

Initial Contact

Call 01270 358000 to speak with our referrals coordinator, or email referrals@elevatedlivinggroup.co.uk. You can also use the enquiry form on our contact page.

We're available Monday to Friday, 9am-5pm. For urgent or emergency placements, our duty line is available outside of office hours - call 01270 358000 and follow the prompt.

Initial Consultation & Availability Check

Our referrals coordinator will speak with you about the child's needs, background, and current circumstances to assess whether we have a suitable placement and the right environment to meet their needs. This is a confidential, no-obligation conversation.

We will confirm availability and provide an initial indication of whether the placement is viable, usually within the same call or email exchange.

Information Sharing & Matching Assessment

If there is a potential match, we'll ask you to share the child's key documentation (see our information checklist below). Our Registered Manager and clinical lead will review the information and complete a placement matching assessment.

We aim to complete our matching assessment and provide a formal response within 1 working day of receiving complete referral information.

Placement Agreement & Transition Planning

If the placement proceeds, our commissioning team will agree terms with your authority's placements team and a contract will be put in place. We'll collaborate on a transition plan - which may include an introductory visit to the home, a phased move, or in urgent circumstances, an immediate admission.

On arrival, the child will be welcomed by their key worker and introduced to the home, their room, and the other young people at their own pace.

Ongoing Review & Communication

From day one, we maintain regular communication with your authority. You'll have direct access to the child's Registered Manager and keyworker. We participate in all statutory reviews, contribute to care plan updates, and provide prompt notification of any significant events.

Referral Information Checklist

To complete our matching assessment as quickly as possible, please have the following information ready. We can begin with a phone conversation and gather documents afterwards.

  • Primary care needs / presenting difficulties (EBD, learning disabilities, etc.)
  • Current placement (if any) and reason for move
  • Placement history overview

Evidence of Progress

We measure outcomes rigorously and can evidence the progress children make during their placement with us - across education, health, emotional wellbeing, and independent living skills. Our outcomes data is available to commissioning authorities on request. See our outcomes page.

We accept referrals for children and young people aged 7-17. If you are uncertain whether a child falls within our remit, please call us - we are happy to discuss on a case-by-case basis.

Yes. We consider emergency and short-notice placements where a suitable space is available. Please call our duty line on 01270 358000 for urgent enquiries outside of normal hours. We will always try to help, and if we cannot accommodate an emergency placement, we will try to signpost you to alternative providers.

We work with local authorities throughout England. We welcome referrals from any local authority - please get in touch to discuss the child's needs. Please call to discuss.

We work with a range of commissioning frameworks and are registered with several local authority preferred provider lists. If you are unsure whether we are registered with your authority, please call us and we can advise. We are also able to work outside of framework agreements via spot purchase, subject to your authority's commissioning policy.

Yes - we actively encourage commissioning authority professionals to visit our residential homes before placing a child. Contact our referrals team to arrange a visit. Where timescales allow, we also try to arrange an introductory visit for the child and their social worker before the move.

We review every child's care plan regularly and communicate proactively with the placing authority about any significant changes in needs or presentation. If a child's needs evolve beyond what we can safely meet, we will discuss this openly and work collaboratively with the placing authority on next steps - always with the child's welfare at the centre.
